Description:

Welcome to our deep dive on interpreting the temporal bone. This session covers radiologic anatomy, pathology synthesis, and diagnostic interventions for advanced clinical evaluation. To start, we explore modality selection protocols. CT is our primary workhorse for bony pathology, while MRI provides targeted evaluation for retrocochlear issues. Stenders Projections assess mastoid pneumatization.Next, our systematic evaluation guides you through the radiologist's checklist. We focus on three key zones: the mastoid region, middle ear, and the inner ear and petrous apex.Let's look at external ear pathologies. We discuss Aural Atresia, often congenital, and Malignant External Otitis, a severe necrotizing infection. Understanding these is crucial for diagnosis.Understanding the ossicular chain is vital. We illustrate traumatic disruption, which alters the normal 'ice-cone' configuration, and Osteogenesis Imperfecta (Type I), a congenital condition making stapedial crura prone to fractures. Here we examine promontory and fenestral anomalies. Persistent Stapedial Artery is a pulsatile vessel, while Fenestral Otosclerosis involves sclerotic thickening at the footplate edges. For differential diagnosis of a middle ear mass, we consider Cholesteatoma with its erosive bone status, Glomus Tympanicum, which is vascular, and Chronic Otitis Secretions, characterized by granulation tissue.Maintaining mastoid vault integrity is paramount. We differentiate skull base fractures from fibrous dysplasia, which can progressively narrow the EAC and present as a dense 'ground glass' appearance.Neoplasms of the facial canal include Hemangioma, characterized by spiculated bone destruction on CT, and Schwannoma, which causes widening of the facial canal and shows gadolinium enhancement on MRI.Inner ear malformations and morphometrics are crucial. We discuss Mondini and Michel Deformities, and the Pathologic Ruler helps identify vestibular aqueduct abnormalities and internal auditory canal status.
